Answers to common questions about our practice, services, and what to expect
Your first appointment is a comprehensive consultation where your provider will take time to understand your history, concerns, and goals. You'll discuss treatment options together and develop a personalized plan. Plan for about 60 minutes — either virtually or in-person at our Sunrise office.
The fastest way is to book directly through our secure online portal. You can also request through our online form, call us at 954-999-5312, or email compassionatebhc@outlook.com. We'll confirm your appointment within 24 hours. If you were referred through Klarity, you can also book directly through their platform.
New patients should complete our General Intake Form, Controlled Substance Agreement, HIPAA Privacy Notice, and Financial Policy. All forms are available for download on our Patient Forms page. Please bring completed forms to your first appointment or email them beforehand.
